Le score albumine-bilirubine (ALBI) a été décrit pour mieux évaluer les fonctions hépatiques dans le CHC. Le score ALBI en baseline a été précédemment étudié, mais aucune étude n’a jusqu’à présent étudié ses variations après SIRT. Méthodes : Nous avons étudié rétrospectivement les données de patients porteurs d’un CHC et traités par SIRT. Les données cliniques et biologiques ont été récupérées. Les variations de l'ALBI à 6 semaines et 3 mois ont été évaluées. Le critère d'évaluation principal était la survie globale (SG). Le critère d'évaluation secondaire était la toxicité. Résultats : 232 patients ont été étudiés. 63 % étaient classés Barcelona Liver Cancer Clinic (BCLC) C et 43 % avaient une thrombose de la veine porte. Sur l’évaluation de départ, 39 % des patients étaient classés ALBI grade 1, 60% ALBI grade 2 et 1% ALBI grade 3. Le grade ALBI de base était significativement associé à la SG (p = 0.001), avec une SG médiane de 24,3 mois (IC 95% : 21,5- 27,0) pour l'ALBI de grade 1 (n = 89) contre 13,5 mois (IC 95% : 10,7-16,4) pour ALBI grade 2 (n = 139) (p = 0,002). Après analyse multivariée, le score ALBI de base en tant que variable continue ou en tant que score à 3 points s'est révélé être un facteur pronostique indépendant pour la survie après SIRT. Les patients ALBI grade 1 avaient significativement moins d'événements indésirables de grade 3/4 et de décompensation oedémato-ascitique après le premier traitement par SIRT que les patients ALBI grade 2 ou 3. Le grade ALBI à six semaines était significativement associé à la SG (log-rank p≤0,005), avec une SG médiane de 26,4 mois (IC 95%: 20,4-32,5) pour les patients ALBI grade 1 (n = 89) versus 14,3 mois (95% IC: 11,4-17,2) pour les patients ALBI grade 2 (n = 139) et 6,4 mois (IC 95%: 4,1-8,7) pour les patients ALBI grade 3 (n = 3). Les résultats étaient similaires à trois mois. Conclusion : Le score ALBI de base et ses variations après traitement par SIRT sont pronostiques. Background: Selective internal radiation therapy (SIRT) is an innovative treatment of hepatocellular carcinoma (HCC). Albumin-bilirubin (ALBI) score was described to better evaluate liver functions in HCC. ALBI was previously studied at baseline, but no previous study presented its variation after SIRT. Methods: We studied retrospectively data from patients treated with SIRT for HCC. Clinical and laboratory data were retrieved. Variation of ALBI at 6 weeks and 3 months were evaluated. Barcelona Liver Cancer Clinic (BCLC) stage was C in 63%, with portal vein thrombosis in 43%. Baseline ALBI grade was 1 for 39% of our patients, 2 for 60% and 3 for 1%. Baseline ALBI grade was significantly associated with OS (p=0.001), with a median OS of 24.3 months (95%CI: 21.5-27.0) for ALBI grade 1 (n=89) versus 13.5 months (95%CI:10.7-16.4) for ALBI grade 2 (n=139) (p=0.002). Upon multivariable analysis, baseline ALBI score as a continuous variable or as a 3 points grade was found to be an independent prognostic factors for survival after SIRT. ALBI grade 1 patients had significantly less grade 3/4 adverse events and ascitic oedema decompensation, after first SIRT than ALBI grade 2 or 3 patients. ALBI grade at six weeks was significantly associated with OS (log-rank p≤0.005), with median OS of 26.4 months (95%CI: 20.4-32.5) for ALBI grade 1 patients (n=89) versus 14.3 months (95%CI: 11.4-17.2) for ALBI grade 2 patients (n=139) and 6.4 months (95%CI: 4.1-8.7) for ALBI grade 3 patients (n=3). Results were similar at three months. Conclusions: Baseline ALBI score and variations of ALBI are prognostic after SIRT.
