Nous retrouvons très peu d’études en France sur les programmes de soins. La rédaction du programme de soins est disparate. Les pratiques varient selon les établissements et les praticiens. Le recours au programme de soins est aléatoire selon les territoires. Le but de notre travail était d’étudier l’utilisation et l’intérêt en pratique clinique courante des programmes de soins en comparaison avec les soins ambulatoires libres à l’EPSM Morbihan à la sortie d’une hospitalisation sans consentement. Méthode : Nous avons réalisé une étude observationnelle rétrospective monocentrique à l’EPSM Morbihan à partir des données extraites des dossiers patients informatisés. Nous avons comparé les données pendant l’hospitalisation complète (HC) en mesure de soin sans consentement (SSC) nouvellement initiée entre le 01/01/17 et le 06/05/17 des patients sortis de l’EPSM Morbihan en soins libres après levée de la mesure de SSC (groupe SPL) avec celles des patients sortis de l’EPSM Morbihan en programme de soins (groupe PS). Nous avons ensuite comparé les données des réhospitalisations et du suivi ambulatoire pendant un an après la sortie de l’hôpital dans ces 2 groupes. Le critère de jugement principal était le nombre de patients réhospitalisés dans l’année suivant la sortie de l’hôpital. Résultats : 100 patients ont été inclus, 64 dans le groupe SPL et 36 dans le groupe PS. Le taux de recours au PS parmi les patients hospitalisés en HC en SSC initialement était de 36%. Les patients du groupe SPL étaient significativement davantage admis en HC en Soins Psychiatriques en cas de Péril Imminent (SPPI) (21.9%) que le groupe PS (2.8%), (p=0.09). Nous retrouvions dans les diagnostics principaux, significativement plus de F20-29(schizophrénie, troubles schizotypiques et troubles délirants) dans le groupe PS (69.4%) que dans le groupe SPL (39.1%), (p=0.004). Le diagnostic F32 (épisodes dépressifs) était significativement plus présent dans le groupe SPL (32.8%) que dans le groupe PS (0.0%), (p= 2.300E-5). Nous ne retrouvions pas de différence statistiquement significative entre le groupe SPL (31.3%) et le groupe PS (38.9%), (p=0.439) concernant le nombre de patients réhospitalisés dans l’année suivant la sortie de l’hôpital. Conclusion : Nous pointons la nécessité de nouvelles études en France. L’élaboration de recommandations pourrait permettre d'optimiser l'utilisation et l’efficacité du programme de soins pour les patients, les praticiens et les équipes. Introduction : The Law of july 5th 2011, revised on the 23rd of september 2013, introduced involuntary outpatient commitment, also known as Care Programs (CP), which were further redefined in the law of January 26 th 2016. There are very few studies about the CP in France. The use and details of the CP varies between hospitals, practitioners and territories. The aim of our work was to study the use and the interest in common clinical practice of the CP compared with free ambulatory care in a population of patients discharged from hospital after an hospitalization without consent. Method : We conducted an observational, single center retrospective study at the Morbihan’s public mental health institution (EPSM Morbihan) We extracted data from the patients‘s computerized files. We recorded inpatient’s data for patients under hospitalization without consent newly initiated between 01/01/17 and 06/02/17, and compared the data of patients discharged on ambulatory free care (SPL group) with those of patients discharged on CP (PS group). We then extracted the data from further hospital stays in both groups as well as data from the outpatient follow up up to one year after initial discharge, and compared it between groups. Our primary outcome was the number of patients rehospitalized in the year after initial discharge. Results : We included 100 patients, 64 in the SPL group and36 in the PS group. The proportion of CP among the patients under care without consent was 36%. The proportion of patients initially hospitalized without consent in the SPPI legal mode (psychiatric care in case of imminent danger) was significantly higher in the SPL group (21.9%) compared to the PS group, (p=0.09). There was more F20-29 diagnosis (schizophrenia, schizotypal and delusional disorders) in the PS group (69.4%) compared to the SPL group (39.1%), (p=0.004). However there was more F32 diagnosis (Depressive episode) in the SPL group (32.8%) compared to the PS group (0.0%), (p=2.300E-5). There was no statistically significant difference in the number of rehospitalized patients in the year after initial discharge between the SPL group (31.3%) and the PS group (38.9%), (p=0.439). Conclusion : More studies are needed in France. Guidelines could optimize the use and efficiency of the involuntary outpatient commitment called CP for the patients, doctors and care givers.
