C’est à partir de ce constat que le principe d’impartialité a été réaffirmé dans les conventions de Genève en 1949. Depuis 2011, en conséquence d’une violente répression des mobilisations pacifiques, la Syrie a sombré dans la guerre civile. La polarisation initiale du conflit entre pro- et anti-Bachar Al-Assad oblige tous les acteurs, y compris soignants, à choisir un camp. Avec l’évolution de la crise, l’arrivée des ONG en soutien aux civils amène un nouveau principe éthique et un nouveau défi pour les acteurs médicaux locaux : la neutralité.  Objectif : comprendre, à partir du conflit syrien, comment les professionnels de santé gèrent l’interaction entre positionnement politique et impératifs éthiques d’impartialité et neutralité dans leur pratique médicale pendant la crise.  Méthode : Etude qualitative par entretiens semi-dirigés auprès d’acteurs médicaux ayant exercé pendant le conflit. Résultats : En mai 2019, à Gaziantep, 2 salariés étrangers d’ONG et 6 professionnels de santé syriens ont été interrogés. L’étude montre que l’engagement médical constitue un engagement politique à part entière face à la criminalisation de l’aide humanitaire par le régime Baath. Les problématiques éthiques se posent différemment selon les zones gouvernementales, d’opposition ou sous influence terroriste. La volonté d’impartialité est forte et sans tension avec leur engagement chez les professionnels interrogés et devient même moteur de cet engagement. La neutralité semble plus difficile à soutenir par les acteurs locaux car oblige à un désengagement politique. Le ciblage répressif dont ils font l’objet et l’échec des organisations internationales à faire respecter le droit humanitaire contraignent les soignants à négocier leur protection auprès des parties en conflit, parfois au détriment de principes éthiques. Conclusion : Dans un contexte de militarisation du système de soins, les soignants deviennent à la fois acteurs et victimes du conflit et les organisations internationales, de par le respect des conventions de Genève qu’elles engagent, restent les garants indispensables à une application efficiente des valeurs éthiques de nos professions. Context: Armed conflict is a place of ethical testing for health professionals. It is from this observation that the principle of impartiality was reaffirmed in the Geneva Conventions in 1949. With the evolution of the crisis, the arrival of NGOs in support of civilians brings a new ethical principle and a new challenge for local medical actors: neutrality. Objective: to understand, from the Syrian conflict, how health professionals manage the interaction between political positioning and the ethical imperatives of impartiality and neutrality in their medical practice during the crisis. Method: Qualitative study by semi-directed interviews with medical actors who exercised during the conflict. Results: In May 2019, in Gaziantep, 2 foreign NGO workers and 6 Syrian health professionals were interviewed. The study shows that medical engagement is a full political commitment to the criminalization of humanitarian aid by the Baath regime. Ethical issues arise differently in government, opposition or terrorist-influenced areas. The will of impartiality is strong and without tension with their commitment to the professionals questioned and becomes the driving force of this commitment. Neutrality seems more difficult to support by local actors as it forces political disengagement. Their repressive targeting and the failure of international organizations to uphold humanitarian law compel caregivers to negotiate their protection with conflicting parties, sometimes to the detriment of ethical principles. Conclusion: In a context of militarization of the health care system, caregivers become both actors and victims of the conflict, and international organizations, by virtue of the respect of the Geneva Conventions they commit, remain the essential guarantors for the efficient application of ethical values ​​of our professions.
