Nous avons recruté 285 patients de plus de 65 ans adressés pour recevoir une chimiothérapie. Nous avons développé un score de fragilité, HEMA-4 à partir des 4 facteurs indépendants de survie à 1 an (1-y OS) les plus significatifs : troubles cognitifs (MMSE<27), comorbidités (≥2 selon le Charlson comorbidity index), CRP (≥10mg/l) et hypoalbuminémie (<35g/L). Ce score a été validé sur une seconde cohorte. HEMA-4 permet la stratification de la population en 3 groupes pronostiques : bon pronostic (n=141, 1- y OS 74%), pronostic intermédiaire (n=37, 1-y OS 51%) et mauvais pronostic (n=28, 1-y OS 36%). HEMA-4 est un score clinico-biologique simple et reproductible permettant de prédire la survie chez les patients âgés souffrant d’hémopathies malignes traités par chimiothérapie. The aim of this study is to develop and validate a simple score, HEMA-4, to screen frailty and predict overall survival (OS) in the elderly population suffering from blood cancer. We prospectively recruited 285 patients (≥65y) admitted to receive chemotherapy who agreed to a geriatric assessment. Cox proportional hazards models were performed to assess correlations between vulnerabilities and OS. We developed HEMA-4 based on the most powerful prognosis factors. It was externally validated with a secondary cohort. HEMA-4 was created based on 4 independent factors: cognitive impairment (MMSE<27), Charlson comorbidity index ≥2, CRP≥10mg/l and albumin level<35g/L. The population was stratified into 3 prognosis groups of respective 1-year OS: good-74%, intermediate-51% (HR=2.30; 95% CI =1.31-4.05) and poor-36% (HR=3.95; 95% CI =2.23-6.98). The HEMA-4 is a simple and reproductible score that predicts survival among elderly patients suffering from blood cancer referred to receive chemotherapy.
