Elle peut être rapide ou lente. Cet essai clinique randomisé a pour objectif de comparer un système de disjonction rapide (vis Hyrax® de Dentaurum®) avec un système de disjonction lente (Leaf Expander® de Leone Odontec®). Matériel et méthode : 34 patients étaient inclus et répartis aléatoirement en deux groupes. Le degré de version vestibulaire des premières molaires a été mesuré avec le système Trios® de 3 Shape et Dental Monitoring®. Les critères secondaires étaient le débit inspiratoire instantané, mesuré à l’aide du PNIF, et la douleur ressentie, quantifié à l’aide un questionnaire standardisé. Résultats : Les résultats mettent en évidence une différence significative en faveur du disjoncteur vis Hyrax® concernant le degré de version molaire. Il n’y a pas de différence significative pour l’évolution du débit inspiratoire instantané ni pour la douleur ressentie. Cependant l’amélioration de la ventilation est significative dans les deux groupes après traitement. Conclusion : Le Leaf Expander® est une alternative intéressante chez les patients jeunes et peu coopérant. La force développée est suffisante pour déclencher une réponse orthopédique tout en limitant les effets parasites dento-alvéolaire comme la version molaire. Introduction: Maxillary constriction, is treated by expansion. It can be fast or slow. The aim of this randomized clinical trial is to compare a fast disjunction system (Dentaurum® vis Hyrax®) with Leone Odontec® Leaf Expander®. Material and method: 34 patients were included and randomly divided into two groups. The degree of buccal tipping of the first molars was measured with the Trios® system of 3 Shape and Dental Monitoring®. The secondary criteria were the instantaneous inspiratory flow, measured with the PNIF, and the pain felt, quantified using a standardized scale. Results: The results highlight a significant difference in favor of the Hyrax® expanser regarding the degree of molar tipping. There is no significant difference for the evolution of the instantaneous inspiratory flow nor for the pain felt. However, the improvement in ventilation is significant in both groups after treatment. Conclusion: The Leaf Expander® is an interesting alternative in young and uncooperative patients. The developed force is sufficient to trigger an orthopedic response while limiting the dentoalveolar indesirable effects such as the molar tipping.
