L’objectif de cette étude était d’évaluer la faisabilité de cette procédure en ambulatoire au CHU de Rennes. Méthode : Cette étude prospective a inclus des patients dont la prise en charge ambulatoire a été validée conjointement entre le chirurgien et l’anesthésiste. L’ensemble de la prise en charge péri-opératoire suivait les pratiques les plus récentes en chirurgie de PTH en ambulatoire avec: une éducation thérapeutique pré-opératoire, une anesthésie générale, une prophylaxie antiémétique, une épargne transfusionnelle, une analgésie multimodale. Les arthroplasties étaient réalisées par voie antérieure par le même opérateur, sans drainage post-opératoire. Le patient effectuait un lever précoce en salle de surveillance post-interventionnelle accompagné du kinésithérapeute puis regagnait le service d’ambulatoire pour poursuivre la kinésithérapie afin d’acquérir l’autonomie pour le retour à domicile. Le suivi post-opératoire consistait en un appel à J1 et J8 par l’infirmière référente selon une check list pré-établie et une consultation à J45 avec le chirurgien. Résultats : Cinquante huit patients ont été inclus entre novembre 2016 et juin 2018. L’âge moyen de la population était de. 64 ans ± 10,6 ans. Le score ASA 2 prédominait à 64% contre 36% d’ASA 1. Trente-deux patients (55%) ont bénéficié d’une titration morphinique avec une consommation médiane de 3mg [0 - 8]). La réussite de l’ambulatoire était de 98,3% avec seulement un patient hospitalisé pour des nausées vomissements post opératoire. La complication notable de notre étude est la survenue d’une fracture et d’une luxation chez le même patient. Discussion : La complication de fracture luxation est en lien avec une attitude luxant et renforce l’idée de l’importance de l’éducation thérapeutique et de la sélection des patients. Par ailleurs, la réussite de l’ambulatoire et les complications rapportées dans notre étude sont comparables aux données de la littérature étrangère ou française. Conclusion : A ce jour, les résultats sont encourageants et montrent la faisabilité de l’arthroplastie de hanche en ambulatoire au CHU de Rennes. L’éducation des patients ainsi que la collaboration pluridisciplinaire sont au cœur de la réussite de ce projet. Introduction : Total Hip replacement (THR) is a current practice in France with almost 100.000 cases realized in 2015 but only 0.5% carried out on outpatient surgery. The purpose of this study was to evaluate the feasibility of THR in ambulatory settings in a teaching hospital. Method : This prospective study included preselected patients for whom the ambulatory pathway was validated by both the surgeon and the anesthetist. The whole medical care was established following the most recent practice of outpatient surgery with pre-operative therapeutic education, general anesthesia, antiemetic prophylaxis, transfusion sparing and multimodal analgesia. Arthroplasty was realized with an anterior approach by the same surgeon and without post-surgery drainage. The patients walked immediately after surgery with the help of a physiotherapist in the Post-Anesthesia Care Unit (PACU) and went back to the Orthopedic Care Unit with the physiotherapist in order to gain autonomy before discharge. The post-surgery follow-up consisted of a phone call on day 1 and day 8 by the referent nurse with a pre-established checklist and a medical consultation on day 45. Results : Fifty-eight patients were included from November 2016 to June 2018. Patients were 64 +/- 10,6 years old, ASA I or II (36% and 64 %, respectively). The success rate of the outpatient surgery was 98.3% with only one patient hospitalized for PONV. Thirty-two patients (55%) needed a morphine titration with a median consumption of 3mg [0 - 8]. The only serious complication was a fracture and dislocation for one patient. Discussion : The one complication (fracture and dislocation) was linked with a luxation posture and reinforce the idea of the importance of therapeutic education and cautious patient’s selection. Besides, results are satisfying with a success rate of 98.3% of the ambulatory surgery. The reported success rate and complications are similar to literature. Conclusion : To this day, the results are encouraging and show the feasibility of outpatient hip replacement at a teaching hospital. Education of the patients and a multidisciplinary collaboration are at the heart of this project’s success.
