La morbi-mortalité peut être réduite grâce à la vaccination anti-papillomavirus humain (HPV). Cependant, la couverture vaccinale en France reste actuellement la plus basse d’Europe. Certains pays voisins à couverture vaccinale élevée s’appuient sur les infirmiers scolaires. Qu’en pensent les infirmiers scolaires en France et quel est leur rôle concernant l’information sur la vaccination anti-HPV ? Méthode : Une étude qualitative par entretiens individuels semi-dirigés a été menée de novembre 2017 à mars 2018, auprès d’infirmiers scolaires volontaires de l’enseignement secondaire. Une analyse thématique manuelle avec double codage a été réalisée. Résultats : Douze entretiens ont été analysés. La vaccination anti-HPV était exceptionnellement abordée avec les élèves. Les infirmiers décrivaient un manque d’information concernant la vaccination anti-HPV, limitant leur perception et la délivrance d’une éventuelle information auprès de la population cible. Le manque de temps et la multitude de missions inhérentes à leur métier étaient des barrières dans le développement de leur mission de prévention. Leur mission était tributaire de la hiérarchie et des intervenants extérieurs. Ils ont mis en doute la transparence des laboratoires et se méfiaient des éventuels effets indésirables liés aux adjuvants. D’autres vaccins étaient considérés comme prioritaires et les infirmiers estimaient que les jeunes filles ne se sentaient pas concernées par ce sujet par immaturité ou manque d’intérêt. Ils en référaient au médecin généraliste, perçu comme un pivot central de l’information autour de la vaccination. Ils insistaient sur l’importance des parents, considérés comme décisionnaires et la nécessité de les sensibiliser à cette vaccination pour améliorer la couverture vaccinale. Conclusion : Les infirmiers scolaires semblent actuellement peu investis dans l’information sur la vaccination anti-HPV. Une meilleure information lors de leur formation permettrait de les sensibiliser à cette problématique de santé publique. La participation de cette profession en contact direct de la population cible pourrait améliorer la couverture vaccinale. Il serait intéressant d’envisager la mise en place d’actions communes avec le médecin généraliste, qui est perçu comme un acteur majeur de l’information concernant la vaccination. Introduction : Cervical cancer is a public health problem. Morbidity and mortality can be reduced by Human PapillomaVirus vaccination (HPV). Nevertheless, in France, vaccination coverage is currently the lowest in Europe. Some nearby countries with high vaccination coverage rely on school nurses. What do French school nurses think about it and what is their role about HPV information? Method : A qualitative study by semi-structured individual interviews was conducted from November 2017 to March 2018, with voluntary high school nurses. Manual thematic analysis with double coding was performed. Results : Twelve interviews were analyzed. HPV vaccination was rarely covered with the students. The nurses described a lack of information about HPV vaccination, which limits their perception and delivery of information to the target population. Lack of time, variety of tasks related to their own job restrained their prevention mission. Their mission mainly depends of their hierarchy and external speaker. Some of them doubted the transparency of pharmaceutical companies and mistrusted any side effects caused by adjuvants. Other vaccines were considered as priorities and nurses reckoned that young girls didn’t feel concerned by this topic, either by lack of interest or immaturity. They refered to general practitioner regarded as the backbone of vaccination information. They underlined the substantive role of parents, as decision makers and the necessity to increase awareness about this vaccination to improve the vaccination coverage. Conclusion : School nurses seems to be thinly invested with HPV vaccination information. Better information during the educational training could sensitize the school nurses to this public health problem. The participation of this profession who work directly with the target population could improve the vaccination coverage. It might be interesting to create common action with general practitioners, who is considered as a key player concerning vaccination information.
