Leur mortalité est élevée dans les pathologies myéloïdes, la prophylaxie antifongique est particulièrement indiquée. Le posaconazole (PZC) comprimé, antifongique triazolé, a amélioré la biodisponibilité par rapport à la forme buvable, pour laquelle le Suivi Thérapeutique Pharmacologique (STP) permettait d’optimiser la posologie par la mesure des concentrations sanguines. La place du STP pour la forme comprimé a été étudiée : d’après la littérature et le suivi prospectif de patients au CHU de Rennes, il apparait utile du fait d’une forte variabilité pharmacocinétique inter et intra-individuelle du PZC comprimé, de patients maintenant des concentrations infrathérapeutiques, et d’autres étant fortement exposés. Le STP permet ainsi l’adaptation de posologie dans les sous-expositions et les fortes expositions au PZC comprimé, et participe à la personnalisation du traitement antifongique. In hematology, neutropenic patients are at high risk of developing invasive fungal infection. Mortality is important in myeloid hematological malignancies, so that antifungal prophylaxis is especially indicated. Posaconazole (PZC) oral tablet, which is a triazole antifungal agent, has improved bioavailability compared to oral suspension for which the dosage was optimized with Therapeutic Drug Monitoring (TDM) by measuring blood concentration. The role of TDM when using oral PCZ tablet has been studied : according to literature and prospective monitoring of patients in Rennes hospital, TDM appears to be helpful, because of an inter and intra-individual high pharmacokinetic variability, and because some patients may maintain low PZC levels while others are highly exposed. TDM allows adjusting the dosage of PCZ tablet with underexposed or highly-exposed patients and may ensure an individualized antifungal treatment.
