Dans ce deuxième cas, l’intérêt d’un traitement prophylactique par Valganciclovir est incertain. Dans cette étude rétrospective multicentrique, 372 patients CMV R+ greffés entre janvier 2012 et Avril 2015 et ayant reçu une induction non déplétante ont été inclus. A un an, 5/222 patients (2,25 %) avec prophylaxie et 9/150 (6 %) sans prophylaxie, ont présenté une maladie à CMV (différence + 3,7; IC95% -0,5 à 8; p=0,002 pour la non-infériorité). Les taux de rejets ou de survenue d’autres infections étaient similaires. Les survies greffon et patient étaient comparables à la fin du suivi. L’absence de prophylaxie ne semble pas délétère chez les greffés rénaux CMV R+ ayant reçu une induction non déplétante. CytoMegaloVirus (CMV) is the most common opportunist pathogen affecting renal transplant recipients, especially during the first months. CMV seropositive renal transplant recipients (CMV R+) are at intermediate risk for CMV disease, but this risk is enhanced in CMV R+ receiving a T-cell depleting induction, compared to CMV R+ receiving a no depleting induction. In the last group, data are sparse to approve a prophylactic antiviral treatment with Valganciclovir to reduce CMV diseases. In this retrospective and multicentric trial, we included 372 CMV R+ transplanted between January 2012 and April 2015 and receiving no depleting induction. During the first year of transplantation, CMV disease occurred in 5/222 patients (2.25%) in the prophylactic group and 9/150 (6%) in the no-prophylactic group (difference + 3.7; 95%CI – 0.5 to 8; p=0.002 for non-inferiority). Allograft rejection and other infectious diseases rates were similar between the 2 groups. Graft and patient survivals were similar at the end of the follow-up. In conclusion, the absence of prophylaxis did not appear to have a deleterious effect on the CMV diseases in CMV R+ receiving no depleting induction.
