Résumé : Contexte : L’Education Thérapeutique du Patient (ETP) est une composante essentielle de la prise en charge des maladies chroniques, dont la prévalence croissante représente un enjeu pour le système de santé en France. L’accès à l’ETP en soins primaires (SP) reste limité. Une collaboration étroite ainsi qu’une une coordination efficace, entre les différents acteurs impliqués sur un territoire, sont nécessaires. Il est donc important d’identifier les structures capables de faciliter cette approche éducative. Objectif : Connaître de quelle manière la présence du Dispositif d’Appui à la Coordination (DAC) Cap Autonomie Santé (CapAS) dans le territoire Lorient-Quimperlé soutient la mise en œuvre de l’ETP en SP. Méthode : Une étude qualitative par entretiens semi-dirigés a été conduite auprès de professionnels de santé (PDS) et d’actrices du DAC. Le recrutement a été effectué à l’aide de la cheffe de projet de la plateforme ETP du DAC CapAS. Après la retranscription intégrale des échanges, une analyse thématique réflexive a été réalisée pour générer des thèmes pertinents. Résultats : Parmi les 12 intervenants identifiés, 11 entretiens ont été menés entre octobre 2023 et janvier 2024. De nombreux participants indiquent que le DAC CapAS occupe une place centrale dans la coordination territoriale de l’ETP. D’une part, il favorise la montée en compétences des professionnels en ETP et renforce la collaboration interprofessionnelle. D’autre part, cette structure prend en charge la gestion administrative des programmes d’ETP territoriaux et joue des rôles stratégiques, notamment pour promouvoir l’ETP. Par ailleurs, le rôle intermédiaire des coordinatrices de programmes d’ETP en SP apparaît comme indispensable, car elles établissent un lien direct avec le DAC. Conclusion : Le DAC, structure porteuse qui a intégré les missions d’ETP dans son secteur d’activité, est un élément de soutien déterminant en SP. Promouvoir l’orchestration territoriale de l’ETP apparaît comme essentiel, aussi bien pour son développement, sa qualité et son efficacité. Abstract : Context: Therapeutic Patient Education (TPE) is an essential component of managing chronic diseases, whose increasing prevalence represents a challenge for the healthcare system in France. Access to TPE in primary care (PC) remains limited. Close collaboration and effective coordination among the various stakeholders involved in a given area are necessary. Therefore, it is important to identify structures capable of facilitating this educational approach. Objective: Understand how the presence of the Dispositif d’Appui à la Coordination (DAC) Cap Autonomie Santé (CapAS) in the Lorient-Quimperlé territory supports the implementation of TPE in PC. Method: A qualitative study using semi-structured interviews was conducted with healthcare professionals (HCPs) and DAC members. Recruitment was carried out with the help of the project manager of the DAC CapAS TPE platform. After the full transcription of the interviews, a reflexive thematic analysis was performed to generate relevant themes. Results: Of the 12 identified participants, 11 interviews were conducted between October 2023 and January 2024. Many participants indicated that DAC CapAS plays a central role in the territorial coordination of TPE. On one hand, it promotes the skill development of professionals in TPE and strengthens interprofessional collaboration. On the other hand, this structure handles the administrative management of territorial TPE programs and plays strategic roles, particularly in promoting TPE. Furthermore, the intermediary role of the coordinators of TPE programs in PC appears indispensable, as they establish a direct link with the DAC. Conclusion: DAC, a supporting structure that has integrated TPE missions into its sector of activity, is a crucial support element in PC. Promoting the territorial orchestration of TPE is essential for its development, quality and effectiveness. | |||
