Résumé : Introduction: L’infection à Sars-CoV-2 entraîne des syndromes de détresse respiratoire aiguë (SDRA) dont certains sont réfractaires aux traitements usuels pouvant témoigner d’une évolution fibrosante, 10 à 14 jours après l’initiation de la ventilation mécanique. De la méthylprednisolone à forte dose peut alors être débutée en thérapie de sauvetage. Notre étude s’intéresse aux caractéristiques de cette population traitée à la phase tardive du SDRA et à leur devenir. Matériel et méthodes: Nous avons réalisé une étude rétrospective en Médecine Intensive Réanimation aux CHU de Rennes (mars 2020 à décembre 2022) et Grenoble (mars 2020 à avril 2021). Tous les patients présentant un SDRA réfractaire traités par méthylprednisolone 2mg/kg ont été inclus. L’objectif principal était de comparer les caractéristiques des patients vivants et décédés à J60 de l’entrée en réanimation et de définir d’éventuels facteurs associés à la mortalité et à la réponse aux corticoïdes. Résultats: Soixante-sept patients ont été inclus. 4,5% des patients recevaient une corticothérapie au long cours. A l’admission l’IGS II médian était à 48 (IQR 33-59) et le SOFA médian à 5 (4-8). 23,9% des patients ont nécessité la mise en place d’une ECMO veino-veineuse. La méthylprednisolone était débutée en médiane 17 jours (12,5-23) après l’admission en réanimation. Les mortalités, hospitalière, à J28, et J60 étaient respectivement de 55,2%, 34,3% et 46,2%. A l’initiation de la corticothérapie, les paramètres ventilatoires (PEP, pression de plateau, PaO2/FiO2, Vte) ne différaient pas entre les patients vivants et décédés. Le rapport PaO2/FiO2 était significativement plus élevé dans le groupe patients vivants à J7 et J15 versus patients décédés (171 vs 123, (p=0,012) et 206 vs 166 (p=0,025), respectivement). Enfin, 63% des patients vivants à J60, étaient extubés avec un nombre médian de jours vivants sans ventilation mécanique de 19 (0-33,3). La prévalence des infections était équivalente entre les deux groupes. La prévalence des PAVM sous corticoïdes était de 30,6% dans le groupe patients vivants, contre 19,4% dans le groupe patients décédés (p=0,44). 33 patients présentaient un évènement thrombo-embolique avec une prévalence équivalente chez les vivants et décédés (52,8% vs 45,2%, p= 0,7). La plupart survenait également avant l’instauration des corticoïdes (16,7 vs 12,9, p=0,93). Les patients répondeurs à J7 et J15 avaient une mortalité hospitalière et à J60 diminuées comparés aux non répondeurs. Conclusion: Les patients vivants à J60 ont un rapport PaO2/FiO2 significativement supérieur à J7 et J15 de l’initiation de méthylprednisolone 2mg/kg. Il ne semble pas y avoir plus d’évènements infectieux et thrombotiques après l’initiation de la corticothérapie. Abstract : Introduction : SARS-CoV-2 infection can lead to acute respiratory distress syndrome (ARDS), with some cases being resistant to standard treatments and potentially progressing to fibrosis within 10 to 14 days after starting mechanical ventilation. In such cases, high-dose methylprednisolone may be administered as a rescue therapy. Our study investigates the characteristics and outcomes of this patient population treated during the late phase of ARDS. Materials and Methods: We conducted a retrospective study in the Intensive Care Medicine departments of the University Hospital Centers in Rennes (from March 2020 to December 2022) and Grenoble (from March 2020 to April 2021). We included all patients with refractory ARDS who were treated with 2mg/kg of methylprednisolone. The primary objective was to compare the characteristics of patients who survived to those who did not survive 60 days after ICU admission, and to identify any factors associated with mortality and the response to corticosteroid treatment. Results: A total of 67 patients were included in the study. 4.5% of these patients were on long-term corticosteroid therapy prior to admission. At the time of admission, the median IGS II score was 48 (interquartile range [IQR] 33-59), and the median SOFA score was 5 (IQR 4-8). 23.9% of patients required venovenous ECMO support. Methylprednisolone treatment was initiated at a median of 17 days (IQR 12.5-23) after admission to intensive care. The in-hospital mortality rates at day 28 and day 60 were 55.2% and 46.2%, respectively. At the initiation of corticosteroid therapy, there was no significant difference in ventilatory parameters (PEEP, plateau pressure, PaO2/FiO2 ratio, and tidal volume) between patients who survived and those who did not. However, the PaO2/FiO2 ratio was significantly higher in the group of patients who were alive on days 7 and 15 compared to those who were deceased, with values of 171 vs 123 (p=0.012) and 206 vs 166 (p=0.025), respectively. By day 60, 63% of the surviving patients had been extubated, with a median duration of survival without mechanical ventilation of 19 days (range 0-33.3 days). The prevalence of infections was similar between the two groups. Among survivors, the prevalence of ventilator-associated pneumonia (VAP) while on corticosteroids was 30.6%, compared to 19.4% in the group that did not survive (p=0.44). Thromboembolic events were observed in 33 patients, with no significant difference in prevalence between survivors and non-survivors (52.8% vs. 45.2%, p=0.7). Most thromboembolic events occurred before the start of corticosteroid treatment (16.7% in survivors vs 12.9% in non-survivors, p=0.93). Patients who showed improvement on days 7 and 15 had lower in-hospital and 60-day mortality rates compared to those who did not respond to treatment. Conclusion: Patients who survived until day 60 exhibited significantly higher PaO2/FiO2 ratios on Days 7 and 15 following the initiation of 2mg/kg methylprednisolone therapy. Additionally, there was no observed increase in the occurrence of infectious or thrombotic events following the commencement of corticosteroid treatment. | |||
